Could someone please advise what is the best way of changing a previously submitted tax return from 2014-15. 

Checking through my previously submitted tax returns, I've noticed my accountant did not claim for foreign tax credit relief on dividend tax I paid at source during that year in Germany. As there is a double taxation arrangement between the UK and Germany, I should have been able to claim 15% back rather than being taxed twice. He only claimed the tax credit as I believe his understanding was you could not claim for both.

Entirely my fault for not being thorough and checking things at the time. However I'm now parting company with him, so would like to know whether I can simply write or phone HMRC to ask for the relief to be added to the old return, or do I have to go through the same accountant who actually made the original return filing. 

Any advice would be appreciated.

You're too late to amend your tax return, and I'm not certain that HMRC would entertain a claim to overpayment relief. But they might, so there's no harm in asking. So, before you part company with your accountant ask him what he proposes to do about his error (if indeed there was an error - there is insufficient detail in the question) - and ask him if his PI cover is up to date.
