Disclosure of change of company name

Sure it is quite simple and all I need to do is put ABC Limited (formerly trading as XYZ Limited) throughout the accounts and then a note in the directors' report stating date of change etc but I can't find anything at CH website to confirm this is correct, nor does my accounts production software make it any clearer.

No - it isn't that simple

Haven't you read the Companies House guidance GP1 Chapter 5?

The company has to pass a special resolution and this has to be notified to Companies House on form NM01 with a fee of £8 using WebFiling or £10 using a paper form in the post.  Until Companies House issues a Certificate of Incorporation on Change of Name, the name has not been changed and it would be entirely wrong to prepare and attempt to file accounts in the proposed new name - Companies House performs very few checks on accounts submitted for filing, but they might well spot this one and reject accounts with a different company name .
