Life assurance with tax relief & Self Assessment

How to claim higher rate tax relief via self assessment on a life assurance with tax relief policy ?

I have an old life assurance term policy with tax relief where mypremium is paid net of basic rate income tax. 

Can anyone tell me where on the Self Assessment tax return I enter details of my premium to claim the higher rate relief please?  I can't find anywhere on the form to enter this type of claim.

Possibly a life assurance policy written as a personal pension contract? You need to look at the detail.

If so same place as pension relief.

Thank you, yes it must be pension term assurance that ended in late 2006. So simply add the gross premium to any grossed up personal pension contribution made then?

Taken out mid-2006. Premium subject to income tax relief but this type of contract was closed to new business from December 2006 but existing contracts are still valid.
I am just not sure how to claim the relief on SA100, as above, I assume the premium is just added to any personal pension gross contribution?
