Just get them to post on Facebook asking their friends for a copy of their own appraisal forms. That way, you'll have not only the questions but also the answers - with no employee or management downtime whatsoever......
As far as I recall, an appraisal will focus largely on the appraisee's agreed objectives for the period under review so I'm not sure what you mean by "questionnaire".
I'd ask your HR manager or failing that get some paid for advice and/or training. There's a lot to lose by not carrying out staff "interactions" properly.
Of course you could also just type "useful appraisal questionnaire" into the Google.

The form should be based around current policies and workplace culture.
It shouldn't be difficult to knock something up that's relevant to your place of work.
It's no good downloading a form such as the one FT has mentioned if it bears no resemblance to your work. For example, if it mentions further training and it isn't something your firm offers, the form will look silly.
An appraisal should be a two way street. Have you asked your staff what they will find useful in an appraisal?

Staff questionnaires have a limited usefulness, due to the fact that most of the staff will just write any old nonsense just to get the form filled because they can't be bothered.
If you do this kind of thing then make sure the process leads somewhere based on the responses or the staff will just get even more p'd off with having to waste time with box-ticking management bullsh*t.
To be honest - I can't say I have ever felt 'motivated' by this kind of HR crap..
I would do your staff a favour and scrap the whole thing.
Why not just talk to your staff to find out how they are?
I agree with Tom and Sosleepy. Appraisals are a complete waste of time and just a box ticking exercise by management. You should talk to your staff on a regular basis not just once a year. That way you will get to know them, what motivates them and how you can help. Form filling is just demoralising. Enough said.
