If a company reimburses mileage at the Approved Mileage Rates, does this need to be shown on the P11d? Or do you have to get a dispensation?

My reading of the IR Mileage Payments suggests that no disclosure is required at all, as long as at or below the rates (40p/25p)

No, but ...
Mileage payments at or below the approved rates don't need to go on the P11D.

However it's still a good idea to apply for an expenses dispensation. If you don't, you'll have to include any other reimbursed expenses related to travel (car park fees, tolls, meals, hotel bills etc.) on the employee's P11D and they'd have to apply for relief to HMRC - a pointless exercise for all concerned!
