Friend of mine, incorporated, declares dividends for the first time to himself, say £6,500.00. Are there any forms to be filled and sent to Inland revenue.

No forms to be filed with Inland Revenue but he should do a minute and create a share dividend tax voucher for retention to prove it was a dividend and not salary.
However your friend should have an accountant to ask - if he hasn't he's going to get into very deep water!
