The original Form 42 requires a declaration and signature at the end. The substitute form on the Revenue website has no declaration or signature line.

Surely the Revenue would want a signature...???

Surprising as it may seem
A signature etc is not needed as what was agreed with the Revenue was the format and the relevant company is identified by the questions on the front page

It will usually be accompanied by the accountant's headed notepaper
