FSA reports on financial services company

I am to shortly meet a prospective new client (introduced by the auditors with a view to preparing monthly management accounts only).

I am aware that a report has to be submitted to the FSA, I believe, at year end and a six monthly interval. Can anybody advise the content of this report/where i may find a template as it would be helpful to have some insight before our initial meeting.
