Bit OF Both
A lot of payroll is admin - and that won't change if you have 1 employee, or 100 employees ( ie sending payslips, advising of IT & NIC liability and etc).

At my last firm we had a minimum charge for I think up to 10 employees.

Anything above 10 had an additional cost per payslip.

This way you should recover any time/costs for the stuff that has to be done regardless.

Per Employee
I find it is best to be specific in your charges and charge per employee say £££ per entry but thats my prerogative.

It really depends on what is easier and best suited to yours and the clients needs.
