Should benefits-in-kind be added to gross earnings in determining the £50k child benefit threshold? HMRC literature does not make this clear.

Yes.

Yes - see http://www.hmrc.gov.uk/adjustednetincome/#2 where there is a list of items to include in Adjusted Net Income - and states "include any company benefits".
