I set up my consultancy company in February 2010 and prepared accounts up to 31 march 2011. My last invoice was in march 2011.

Since then I have been unwell but hoped to start work again. Unfortunately it now looks unlikely that I will start work any time soon so I am looking at closing down the company.

My question is what co house form do I need to submit. Plus I presume I need to let hmrc know too?

Form DS01

If you want to initiate the process, you need form DS01; there is a CH fee of £10.

Or you could just not file any further Annual Returns or accounts and wait for CH to initiate the process.

You do not need to notify HMRC as such, but it might be expedient to write a letter saying that the company ceased trading on 31st March 2011 and has paid all the tax on its profits, so please do not to object to the striking off when CH publishes the required 3 months notice.
