Directors' wife purchased the van and raised an invoice on the Company.
The Company has one director who carries our deliveries from their home which is their registered address
Does the V5 needs to be in the company name as well ?
The van is used for business and located at their home and they have a car for personal trips. Could HMRC still say there is a VAN benefit charge ?

Although the query is still confusing ...
... and lacking in pertinent information I'll hazard a guess and say that there should be no van benefit.
V5 registration document
The V5 records the registered keeper of the vehicle, not necessarily the owner. If the van is being used by the husband, perhaps it should be in his name, rather than his wife's.
There is no van BIK if the van is provided by the company to the husband mainly for business use and is used only for business travel and commuting (no scope for that as his home and workplace are the same) with no significant private use. There is no basis for a van BIK on the facts stated.
