One of our clients is paying for his mothers private medical insurance as a company contribution, even though she is not an employee

Does this go on his P11d as his mother is not an employee?

It depends...

... on the reason for the company paying it.

If it is being paid by reason of the son's employment/directorship with the company, then John is, of course, correct.

If it is being paid by reason of the mother's former employment with the company or by reason of her being a shareholder in the company, then there's an altogether different answer.
