HMRC sending personal info to wrong place

I have today received a CIS check letter for 2011 for one of my clients from HMRC.

HMRC have kindly included a list of captured vouchers - for someone else!  Not one of my clients.

Do I therefore assume that my clients UTR, Name and earnings have been sent somewhere else?

Can anyone suggest how I should approach HMRC regarding this.  Does this breach data protection act?

Yes it almost certainly does breach the Data Protection Act.
