I am sure it used to be possible to apply for Flat Rate VAT registration using an HMRC online system, but this is not visible in any of the HMRC Online pages.
Alternatively it's only available online if you are making an initial application.
So why can't they get their act together and just add it to the options within the VAT section of the HMRC Online services.
Same goes for RTI setup - there should be something within the HMRC Online / PAYE section ...

Not sure it was ever possible online
I may be wrong but I don't think it was ever possible to file an application online. It is possible however to download the form VAT600FRS from HMRC website, complete the form on your screen and send it by email to [email protected] (which is almost the same thing).
As for RTI, you don't need to set it up. It's just something that has to be filed online; either with commercial software or by using HMRC's own tools.

I've never seen FRS as an option for the new VAT registration application. I think HMRC want to keep the application as simple as possible.
The last time I did one, a month or so ago, the option was there on online registration.
That was in the public registration screen, not the agent specific one (which I've never got my heard around)

It was a pleasant surprise to see it.

It is there as part of the "register for HMRC taxes" when you go through the VAT regn, as you near the end it asks if you would like to apply for VAT schemes (Flat rate etc)
With regard existing registrations I usually do over the phone, quickest way and the advisor takes the details and submits quickly. (although seeing the above link from SteveOH I might email in future)
i think it depends on what you put in as turnover
if its above a certain figure , tho not the treshhold, then it bypasses FRS questions. I regard this as shameful
It's definitely there for new registrations - it's a supplementary question at the end of the registration process (but I haven't done any that would take it over the threshold so maybe that would skip it as mentioned above).
On a related but different note, I've had no end of problems with an online de-registration going astray. Ended up having to contact the de-reg unit directly to manually pull it out and re-enter - 5 weeks after the initial online request. Every time I spoke to someone, they confirmed it was "in the system" - just not moving very fast.
