does private medical insurance for the directors and their families need to be shown as a separate expense in the accounts? Client has included it in insurance account. thanks.

No need for a separate line, it's OK in insurance. Presumably it also gets picked up as a BIK for directors' emoluments and P11D.
It doesn't need to be a separate line in the expenses, but it can be if you want. The advantage of it being on a separate line is that can be more easily picked up for the purposes Terry mentions. On the other hand you probably don't want a proliferation of extra lines unnecessarily - and if you start here where would it end?
