It should really integrate with the accounting system
As a stand-alone system you could make it as simplistic or complex as you wanted. There's lots of stuff on Google here's an example.
You don't say what size business it's for but, these days, it would be usual to use an accounting system that incorporated a purchase order system, or for more advanced stuff, to use PO software that integrated with the bookkeeping. Again lots on Google.

We usually bifurcate PO in 4 types- (i) Capital (ii) Revenue (iii) Work orders and (iv) Technology transfers. Each type of order is started with a different number. It will be better to identify each item ordered with a unique code no. based upon its specification and quote this number in the PO. We may codify other things also like suppliers, major terms of PO etc. also for quick understanding. As regards maintaining its record, Mr. Paul Scholes has beautifully answered.
