You can apply for it retrospectively.

Form

You have to fill in the refund form rather than the exception form. I'm not sure whether you have to actually physically pay it before the refund can be granted though.

The form to which you refer is now in 'archive' and is un-usable, according to HMRC.

I've used it to apply retrospectively a number of times for very small clients - 

but I found it to be archived now - and thought there might be another route
