My client lost her husband in November 2005 and we informed the Revenue of his death and her widowhood in writing in the December

So far, we have had

i) A 2005/06 return sent to him rather than his executors (one of the partners here)

ii) Above return queried as to why he hadn't signed it (my response to Revenue being 'Because he's dead!')

iii) Refund sent to him rather than to executors as requested on TR, which had to be returned as clearly client could not cash it.

iv) And the latest - 2007/08 coding issued to widow including married couple's allowance (she always had it previously as her income was higher).

Client is becoming understandably distressed. Our last letter requesting an amdendment to the coding stated that the Revenue had now been advised of the husband's death FIVE times.

In a nutshell, write a complaint letter to the office, then write one to the director of the office saying you are still not happy.

They shouldn't be making these mistakes, especially in such a delicate matter as death. However in their defence we have had to notify them a fair few times about client deaths and to my knowledge have never had to tell them twice, so you may have just got a dozy office/team.
