I have entered my details and waited for the phone call, which has had a long delay (in one case 10 minutes) when I answer the call there is silence before the line then goes dead. I have tried several times without success.

Yes, I am also experiencing the same problem this afternoon.
After the first silent call that I received from HMRC I then received a further three silent calls in quick succession (all logged as incoming from 0300-200-3600)
I've spoken with HMRC online services but they were no help at all (what a sruprise !) The woman I spoke with said she'd had one other person call in with the same problem and the only advice she could offer was to report the fault via the 'get help with this page' link on the HMRC login page requiring the access code.
Since phoning the helpdesk I have tried to login again, but this time not even a silent call back.
Is suspect this is just a foretaste of the inevitable MTD train crash that is heading our way.....!!
The interfaces for sending phone calls are much more complex than those for sending text messages. (I have written both) Hence I would suggest when using two part authentication that people should try to find a way of using a text message where possible (I know that people use the office number).
I have myself found the HMRC gateways really slow, however.
I logged into my own without any calls/texts - went straight in as it used to do before they introduced the daft security process!
Weird
I have just had a text HMRC access code but I have not been trying to login! Not sure whether someone is trying to access my account or whether it is one of the ones you are missing!
Edit: Ah - mystery solved. Someone was trying to access the VAT account of a not for profit business that I am a volunteer on and for which I am the nominated text message recipient. It would be useful if the text message gave a clue as to which account it related to!
