I have ordered three codes recently, one was a second request, but none has arrived at my clients' address. Are others finding this, or am I just unlucky

I assume the request was successful? If so, it suggests HMRC have the right address (or at least postcode). It's possible they may have a different house number/building name. Otherwise, it sounds like you may just be unlucky.
They tend to arrive ten days after the request, make sure you give them this timescale.
A couple applied for recently (1 VAT & 1 PAYE, different clients) have arrived with no issues.
Also just make sure you are aware that the application is a two step process online to get the code sent out. It's easy to overlook the second step.
There are other routes towards being appointed as an agent that should take a lot less time (minutes rather than 10 days). In theory it is possible to be appointed as an agent by someone who has not yet created their own gateway. I have not managed to see that work yet, however.
One is the GUI on the ASA and the other is the Agents API.
I know the validation at HMRC is a lot more picky for this than other systems for obvious reasons as I have had discussions with them about individual cases.

had the same problem recently....in a stupid client vs HMRC...I would still point my suspicious finger in the direction of HMRC....I wouldn't be surprised if they only allow 1 in 3 applications to be issued in order to save on post.....
