Does the GP complete part 1 and send it to the PCT without any numbers?

Or does the PCT complete part 2 and send it to the GP for signature and return.

These superannuation forms are completely unintelligible. The taxes acts read like a novel in comparison.

GP SOLO FORMS
I am a specialist medical accountant so many of my clients have these forms.

The GP SOLO forms are completed by the "employing authority" i.e. the organisation running the out of hours service or the PCT if the SOLO income is appraisers fees etc. This means boxes A to G and usually most of part 1 as well.

The completed form is normally sent to the GP with their cheque for the work done (less the 6% ees contributions in box D). The GP signs the form in part 1 and sends it back to the employing authority who then sign it in part 2 and send it to the PCT with a cheque for the amount in box G.

Main tax return entries
1)What will go to main tax return for solo gp income.
And in which box
2)does pension deducted on that form will go to box 3 tr4 page (both employee and employer)
