Has anyone else had these MTD issues?

We've had two recurring issues with MTD authorisation, has anyone figured them out?

Issue 1:

Client signs themselves up for MTD. We then request authority through our Agent Services Account, but when they click the link it says they aren't authorized to approve and need an individual gateway login.


Issue 2:

Some of our clients are being signed up with for us to handle MTD, and some are only being signed up to Submit VAT Returns. Is anyone aware of how the ASA is different for these, and whether anything needs doing differently?
