On the 'Sign in' page, sign is as normal.

On the next page which requires the access code, below the green 'Continue' button, click on 'Need help signing in?' and this gives the option to change the phone number.

Once registered, if a taxpayer loses their mobile phone or changes their number, they will need to phone HMRC’s online services helpdesk to deactivate the old phone number and will then be asked to re-register the next time they log in.

Thanks ma7306 I have been trying to find guidance on this. Would I be able to call on behalf of my client or will he have to do it.

you should contact HMRC regarding this, they will help you, but if it doesn't solve your problem then ask any expert accountant or talk with an online accountant, like https://www.weaccountax.co.uk/accountants-in-london-uk/
