ICAS requires me to have £100k of PII before issuing the practicing certificate. I've been looking through their list of approved insurers and Axa seems to be coming out at about £130 pa.
DirectLine declined but I think that's because I ticked the box that said I will be providing "tax planning advice that falls within the definition of tax avoidance". I think they're trying to cover the registered schemes with that rather than basic avoidance.
Anyone have any recommendations for good service/fee based on experience?

£100k seems rather low. Based on our expeience an increase to £250k is not a lot more. Have you tried Hiscox?
