HMRC say that RTI is working well.
It turns out that HMRC are not recording eligibility for employment allowance with one of my clients. I was using Sage but now I use BrightPay. I phoned Sage and they checked and they said I'd claimed. I phoned HMRC and they said that there were 5 EPS submitted that were not on my log of submissions.
HMRC seem to be accepting submissions from anybody or there's some other cockup.

I am shocked at this post.
You are insinuating that HMRC aren’t very good. But in another thread, you expect that digital accounts will be a breeze for taxpayers and will not be a burden at all.

I asked them to record who sent the rogue EPSs. They said they don't think they can get that information. I pointed out that is a very important security failing!

We had the same problem when we moved payroll software, we had to tick the EA box again on the first submission from the new software even though it had already been ticked at the beginning of the tax year. It was rectified by ticking the box on the next submission. Could this be the problem?

Software doesn't work unless people make it work. And they have insufficient people working.
They're going to need a lot of staff in their call centres come 2018.
