I have had an email from HMRC showing a RTI failure and obviously quoting the PAYE ref. Having searched for the PAYE ref it doesn't seem to be our client.
I can only conclude that someone is using our log in and not on our server which is possible but unlikely.
HMRC obviously will not tell me who the employer is with this PAYE ref as I am not the agent, however they are prepared to cancel it.
Anyone know of any other scenarios that could have happened. I am aware that agents unknowingly pass their user id and passwords onto other accountants via payroll back ups however we have been very careful not to do this.
Could another user with a similar user id have made a typo and combined with an incorrect password would have created an unsuccesful submission. But would I have had the email sent to me in this scenario? I guess so?

User id / password / login isn't really relevant.
The emails are sent to whatever email address is set up in the payroll software, not any email address associated with your user id.
Is there a typo at your end?
Have you checked you haven't tried filing with an incorrect PAYE reference?
Does your payroll software indicate any submission failures?
As suggested elsewhere, to reduce anonymous posts, I am not answering this poster's query.
I know from personal experience that HMRC's handling of possible security breaches is inadequate (in fact, non-existent). HMRC needs to be able to act on these reports from agents. HMRC failed to act on my issue so I reported it to ACCA. They escalated it up HMRC to officers of a sufficient seniority for something to be done.
