System for taking card payments over the phone

Hi all
We have now got our card machine ready to rock and roll.  I need to write up some internal procedures for taking credit card payments (don't write the number down etc).  Google search hasn't yielded anything suitable. 
Before I reinvent the wheel, I don't suppose anyone has something already set up that they would be prepared to share. 
Are there any special rules (other than looking after receipts locked away) that I need to be aware of?

I have a feeling that you need to do some sort of back ground checks on those who will be using the card machine.

Read up PCI DSS rules about storage & transmission of card information (basically don't do it unless you have to)

And why a card machine? Unless you have people tuning up with an actual card to pay, why not use a virtual terminal (better audit trail, >1 person can use at same time etc)
