I would definitely say you need a website. A lot of people when looking for accountant will search for exactly that. Or if you gave somebody your contact details and they somehow misplace this, your website will be another way of obtaining this contact details.
How did the client pay for all the expenditure? If they used a debit or credit card, you can go through this and pick up all the payments and keep this statements as evidence, but you need to strongly stress to client they need to keep all the receipts in future.
I agree with the others, if client provided you with these bank details. And they confirmed you can make the repayments to that account, you have done your part. Normally each year when there is a refund due, we tend to ask clients which account they need the repayment sending to, even if we hold previous bank details.
This is very tricky, but like a lot have said, if the clients decided to follow you on their own will,then there is nothing much you can do, but to be honest i don't think your previous employer will take it very kind.
Claiming actual cost could be tricky and also attract benefit in kind, hence you have to be careful on this. Best to stick to paying the mileage. If the company only pays lets say 35pence per mile, then the employee can then claim the difference 0.10
The other thing you will need to consider, if you bought the practice, will all clients agree to work with you, as sometime you tend to see when a practice is sold, some clients wants to make the choice of choosing another accountant. What you don't want is paying for this sort of clients and then after you have paid they end up looking for another accountant.

Its always good idea to have the dispensation in place, this at least covers you. Without dispensation you will be expected to file the P11d;s.
