I struggle to recommend Receipt Bank as a solution to clients. We trialled the service for a while and ended up getting charged during that period and the customer service was horrendous. On top of that the software is not infallible.
I understand that while this guy was supposedly Self Employed the contractor (Pimlico Plumbers) did not permit him to take jobs outside of that firm which was a strong element of the judgement being passed.
In my opinion the initial closure of contact at local offices and the move to centralisation over ten years ago was the start of the current decline in service levels at HMRC.
Given the flawed implementation of the RTI system (which in theory is a great idea however flawed in execution) I have no confidence at all in HMRC's ability to roll this out successfully.
My answers
Can't access due to capacity issues.

Joint Bank Account
I assume that payment of dividends into a joint bank account would be ok? For example in the name of Mr and Mrs xyz?
Bad idea
In my opinion the initial closure of contact at local offices and the move to centralisation over ten years ago was the start of the current decline in service levels at HMRC.
Crystal Ball
I foresee a 12 month contracting period and reduction of the 24 month Travel and Subsistence for temporary workplaces to 12 month's also.

Penalties
The size of the penalty varies depending on the number of employees.
1 to 9 is £100
10 to 49 is £200
50 to 249 is £300
250 + is £400
Controversial Alternative
It would never happen but the alternative would be for the Government to provide tax relief on the cost of commuting to work.
General Advice
I think for general advice this could be a good tool. Certainly seen worse ideas from HMRC.
