I registered as agent for my own returns some years ago following a tip on AWEB that any queries could get a quicker response on the ADL (?!! it was some years ago!).
In my experience the refund is frozen and it is necessary to ask HMRC to consider disregarding the old debts and releasing the refund. Takes a while, call the agent helpline and they pass on the request to a department that cannot be contacted directly. The latter may then ask for any outstanding returns to be submitted, on paper if necessary.
My fees are on my website too, but over the years I estimate at least 50% of enquirers tell me that they have only looked at the search result to get my phone number and have not clicked through to the website!!
Regarding the subsistence payment, my understanding has been that an expense is claimable for occasional travel but not when an integral part of the job is daily travel - on the grounds we have to eat whether we are working or not. How wrong am I?
The agent helpline have told me there is a known error where a SATR is received but not processed. They then need to ask the IT crowd to nudge it on. I am still waiting after several weeks. It was explained to me as a comms problem, not connected with the content of the return. I don't know if that is right but I now go back in to check the status of every return I make.
Specifically to CIS deductions suffered, if the subcontractor's SA tax return figure does not match HMRC's figures from the contractors they will follow this up. I imagine this is an automated process.

To be fair, wouldn't HMRC have to employ somebody full time just signing for post? I gave up on recorded delivery to them many years ago!
