Every time you have to correct an HMRC error ask them for £10 towards your clients costs. Say that if they aren't prepared to pay it can they refer the matter to the national complaints team for consideration
Complain to your client's MP MP complaint cases may still get prioritised although with another £80M of staff cuts things will be getting worse very quickly.
Complain about the former agent - surely one should reasonably expect a professional accountant to help identify ways of saving you money. I'd also complain to HMRC as to why they've required a return all of these years. They are supposed to only require them when necessary and by asking for them each year they've generated a lot of cost
