The case of the duplicate NI number is an illustration of something HMRC says is impossible - ie it can't happen, but it does. When I was in practice (over 20 years ago) I had a similar situation with one of my cleints, who I beleive was called O'Keeff.
It also highlights a fundalmental problem with the quality of payroll data held by HMRC, and is a clear warning sign of what could happen with MTD.
Apparently, according to HMRC and most gov departments, they do not make mistakes. It is the taxpayer, the patient, the GP, the hospital or the claimant, but never them.
I had a mixed records case and it took HMRC over a year to admit they had scrambled the records of my client with those of someone with the same name living 200 miles away.
These cases demonstrate the cultural arrogance embedded in HMRC from top management down to the newest recruit - once he or she has been trained in the HMRC way of doing things.
It's this finger-pointing attitude, everyone else to blame except them, that in my view is really going to explode in their faces when MTD kicks off.
Everyone sensible has told them what they need to do for successful implementation and they have ignored everyone. The problem that gives them is that their finger-pointing will not wash when it blows up, it will be 100% an HMRC muck-up and seen to be theirs.
